Our Going Home program reconnects newly unhoused or stranded individuals with loved ones who are willing to take them in until they can become stable and self-sufficient. Our team negotiates a social contract to help rebuild stable relationships, covering meals & transportation costs and arranging further resources (substance abuse treatment, medical and/or mental healthcare), as needed. Our social worker offers 3 months of post-reunification follow-up. After 3 months of follow-up, less than 8% of people have wound up back on the streets.
